Qualifications

• 2+ years in care management, utilization management, behavioral health, or managed care.
• LPN/LVN license required only if state contract mandates; license must be active in state of practice.
• Strong customer service and interpersonal skills.

Responsibilities

• Monitor the intake support center and care coordination portal daily.
• Ensure stakeholders and members have access to portal information.
• Triage member inquiries and route to appropriate departments.
• Serve as point of contact to external care coordination entities and providers.
• Facilitate member referrals as needed.
• Attend training on care coordination portal use and access.
• Support population health management and related processes and services.
• Identify operational issues and report to leadership.
• Collaborate with internal/external partners to execute program requirements.
