Qualifications

• At least 2 years in care management, utilization management, behavioral health, or managed care.
• LPN/LVN license active and unrestricted if license is required by state contract.

• Home office with high-speed internet connectivity.
• Effective verbal and written communication skills.
• Proficiency with Microsoft Office or applicable software.

Responsibilities

• Monitor intake support center and care coordination portal daily.
• Provide access to care coordination portal information for stakeholders and members.
• Triage member inquiries and address concerns promptly.
• Serve as point of contact to external care coordination entities and members.
• Facilitate member referrals as needed.
• Attend training on care coordination portal use and access.
• Support reporting requirements to address member needs.
• Collaborate with partners to execute program requirements.
• Monitor program metrics and drive process improvements.
• Build partnerships with community-based organizations.
