EverCare Mobile Health is a community-based behavioral health agency focused on improving mental health, independence, connection, and quality of life for individuals living in residential and long-term care settings.
We bring support directly to clients where they live and work closely with residents, families, facility staff, and behavioral health providers.
As an EverCare Care Partner, you will provide relationship-centered, non-clinical support to individuals living in residential and long-term care environments.
Your role is centered on connection, engagement, independence, and quality of life.
The Care Partner is a non-clinical supportive role.
Care Partners do not independently diagnose, conduct clinical assessments, provide psychotherapy, create treatment plans, make medication recommendations, or provide credentialed behavioral health services.
The position does not include skilled nursing, medication administration, or hands-on personal care unless specifically assigned and the employee is appropriately qualified and trained.
Preferred: Experience in assisted living, memory care, skilled nursing, behavioral health, human services, home care, recreation, or working with older adults or individuals with disabilities. CPR/First Aid certification preferred.
Successful Care Partners show up consistently, build genuine relationships, communicate concerns promptly, and find creative ways to help clients experience greater connection, independence, engagement, and joy.
Sometimes making a positive impact starts simply: listening, keeping your word, sharing a laugh, and helping someone feel genuinely connected.

Flexible work from home options available.
