EverCare Mobile Health is seeking a Care Partner to provide relationship-centered, non-clinical support for residents in residential and long-term care settings in Idaho Falls. You will focus on connection, independence, and quality of life, building trusted relationships and engaging clients in activities.

The role involves traveling between client locations, maintaining confidentiality, and collaborating with facility staff.

Qualifications

• Strong interpersonal and communication skills.
• Dependable and able to maintain a consistent client schedule.
• Ability to pass Idaho background checks and facility clearances.
• Valid driver’s license, reliable transportation, and auto insurance.
• Comfortable working independently in community settings.
• Basic smartphone and technology skills.
• Preferred: CPR/First Aid certification.

Responsibilities

• Provide relationship-centered, non-clinical support to individuals in residential and long-term care settings.
• Build consistent, supportive relationships with assigned clients.
• Provide companionship, encouragement, conversation, and meaningful social interaction.
• Engage clients in recreational, social, wellness, and community activities.
• Support outings and activities and encourage independence and engagement.
• Observe changes and communicate concerns to team members.
• Maintain professional boundaries and HIPAA compliance.
• Travel between assigned client locations throughout Idaho Falls area.
