Job summary

Caring for Portland, LLC seeks a Care Manager to act as the central hub coordinating care, compliance, staffing, scheduling, and client communication across our in-home care teams. You will develop service plans, oversee authorizations for billing, and manage after-hours support to ensure seamless client care.

You will also ensure branch operations meet licensing and HIPAA standards, while delivering high-quality service to Clients, Families, and Caregivers in a fast-paced environment.

Qualifications

• Experience in home care or healthcare coordination.
• Strong written and verbal communication in English.
• Attention to detail and accurate record-keeping.
• Knowledge of HIPAA and PHI handling.

Responsibilities

• Provide 5-star service to all incoming calls as a hub for Clients, families, Caregivers, Case Managers, and referral partners.
• Coordinate care services to ensure timely and consistent service delivery for all Clients.
• Develop individualized service plans and conduct initial and ongoing Client assessments for quality and compliance.
• Prepare Client welcome materials, intake documentation, and manage authorizations to support billing accuracy.
• Support scheduling and staffing to minimize overtime and ensure shift coverage.
• Provide after-hours support to maintain seamless care continuity.
• Maintain Branch compliance efforts, including licensing documentation.
• Conduct weekly New Hire Orientation sessions for incoming Caregivers.
