Job summary

Prosano Health Solutions, in partnership with Blue Cross Blue Shield of Arizona, is hiring for an on-site care support role in Arizona. The role focuses on educating members, coordinating referrals, and supporting chronic care management to improve health outcomes.

You will work with the Care Team to manage referrals, schedule appointments, and ensure a high-quality patient experience across Phoenix, Tucson, Chandler and Flagstaff. Prior healthcare experience and EMR proficiency are valued.

Qualifications

• Excellent interpersonal and communication skills with patients and team.
• BLS certification is required.
• High school diploma or GED is required.
• Interest in continuing education/certification.
• EMR experience and ability to handle confidential information.

Responsibilities

• Support members in-person and virtually with education and scheduling.
• Coordinate referrals to specialists and diagnostics with follow-up.
• Contribute to improved health outcomes and cost savings.
• Serve as a visible and empathetic partner to members.
• Explain referrals and manage the end-to-end referral process.
• Assist in quality programs and patient outreach to close care gaps.
• Support members with chronic conditions through reminders and education.
• Lead service and schedule requests on behalf of members.
• Provide referral information to Care Team and assist in specialist relationships.
