Job summary

DCH Health System is seeking an experienced Care Manager to coordinate patient care across acute and post-acute settings. You will partner with physicians, social workers, and care teams to optimize transitions and ensure efficient, patient-centered care plans.

Responsibilities include timely assessments, utilization monitoring, and collaboration to reduce readmissions, with a focus on safe discharge planning and cost-effective care delivery.

Qualifications

• RN licensure in Alabama.
• BSN or related bachelor’s degree preferred.
• Experience in utilization and/or case management preferred.

Responsibilities

• Complete face-to-face assessment of all new patients within 24 hours or next business day to identify appropriateness for acute care and care planning needs.
• Collaborate with multidisciplinary team and patient/family to execute plans and interventions to facilitate stay and manage length of stay.
• Participate in multi-disciplinary rounds and care conferences to promote comprehensive, coordinated care plans.
• Coordinate with social workers for complex needs; arrange home care, DME and post-acute services as needed.
• Provide counseling and support; obtain guardianship and palliative consults when appropriate.
