Qualifications

• Must have state RN/LVN/LMSW or LCSW licensure.
• Experience in discharge planning and utilization management is preferred.
• Must be able to work independently and exercise sound judgment.

Responsibilities

• Meet expectations of OneCHRISTUS competencies (Leader of Self/Others/Leaders).
• Interview patients/families to assess social, emotional, and financial factors affecting health status.
• Develop and monitor patient plans of care with CM II/III for effectiveness and service appropriateness.
• Coordinate care progression across continuum in a cost‑effective manner.
• Advocate for patients regarding treatment decisions and end‑of‑life issues.
