Job summary

CHS Career Site is seeking a Care Management Specialist to provide administrative and operational support to the Care Management team. The role coordinates discharge planning, insurance verification, and documentation management to ensure a smooth transition of care and timely access to resources.

You will work closely with Care Managers, Social Workers, providers, and patients, collecting information, coordinating services, and improving continuity of care.

Qualifications

• 1–2 years of experience in care coordination, case management support, patient access, or healthcare administration.

Responsibilities

• Assist with discharge planning, referrals, and follow-up services for post-hospital care.
• Verify coverage with insurance providers and obtain necessary approvals for care services.
• Maintain documentation in the EMR with notes, referrals, and authorizations.
• Support patient and family communication about discharge instructions and follow-up appointments.
• Coordinate with home health, DME, SNFs, and other post-acute services to ensure smooth transitions.
• Review patient charts and forms to ensure information is available for staff.
• Track pending authorizations and service requests, escalate as needed.
• Assist with patient screenings and assessments per regulatory guidelines.
• Incorporate age-specific discharge planning considerations to meet patient needs.
• Maintain compliance with CMS, Joint Commission, and other regulations.
• Perform other duties as assigned and maintain reliability.
