Qualifications

Responsibilities

• Collaborates with patient/family and multidisciplinary team to plan care from admission through discharge.
• Identify and manage patient flow to optimize throughput and length of stay.
• Develop and monitor plans of care and respond to barriers impacting care.
• Provide discharge planning and care coordination across settings.
• Document in medical records per guidelines and ensure regulatory compliance.
• Participate in multidisciplinary rounds and communicate with payors and physicians.
• Assist with transitions to post-acute care and community resources.
