You will assess needs, develop plans of care, coordinate discharge planning, and advocate for optimal resource use and patient safety.

  • Interviews patients/families to assess social, emotional, and financial factors.
  • Develops and monitors the patient care plan with CMII/CMIII.
  • Coordinates care progression through the continuum of care.
  • Escalates cases to Physician Advisors per policy.
  • Documents in the medical record per guidelines.
  • Participates in Multidisciplinary/Patient Care Progression Rounds.
