Job summary

The Chautauqua Center, Inc. (branded as TCC Health) is seeking a Care Coordinator to provide coordinated care for patients by developing, monitoring, and evaluating interdisciplinary care plans with input from physicians, nurses, social workers, and other members of the care team.

You will orient patients and families, arrange referrals, perform home visits when needed, advocate for patients within the healthcare system, and participate in Patient-Centered Medical Home activities to improve

Qualifications

• Bachelor's degree preferred or combination of Associates degree with relevant experience.
• One year ambulatory or public health experience preferred.
• Excellent interpersonal and communication skills required.

Responsibilities

• Orients and educates patients and families and initiates care plans with direct care providers.
• Develops interdisciplinary care plans and coordinates information with other care providers.
• Evaluates care outcomes with the team and implements recommendations.
• Maintains patient confidentiality and quality service standards.
• Assists patients through the healthcare system as a navigator and advocate.
• Completes home visits to evaluate patient needs and living conditions.
• Initiates referrals for services and participates in quality improvement initiatives.
• Participates in Patient-Centered Medical Home team meetings.
