Qualifications

• 4+ years in care navigation or related outreach roles in healthcare.
• Ability to work independently in a high-volume environment.
• Strong communication and empathy with patients.
• Experience addressing social determinants of health (SDoH) preferred.
• Bilingual Spanish a plus.

Responsibilities

• Conduct high-volume outbound calls to engage patients and schedule services.
• Coordinate care across mammograms, bone density tests, and primary care.
• Connect patients to social/community resources to remove access barriers.
• Document all interactions per HIPAA and privacy standards.
• Collaborate with clinical, operations, and support teams.
• Escalate issues promptly and follow up to close gaps in care.
