Job summary

Community Bridges is seeking a Navigator II to support clients by identifying barriers, connecting them with services, and coordinating wrap-around supports. The role emphasizes outreach, engagement, and ongoing case management within a multidisciplinary team.

You will document interactions in line with AAC, ADHS/DBHS, CARF, and internal policies while mapping the best next steps for each individual to achieve sustained success.

Qualifications

• High school diploma or GED is required; Associate degree in Behavioral Health preferred but not required.
• Minimum of six months of recovery from substance use and/or mental health disorders required.
• 1-3 years of full-time Health Care related work experience where exposure to both behavioral and medical crisis situations is preferred but not required.
• 1-3 years of substance abuse/mental health recovery-oriented service is preferred but not required.
• Minimum of ninety (90) days experience in a Navigator or Peer Support Specialist role required.
• Current Driver’s License (Valid and in good standing).
• Clear 39-month Motor Vehicle Record.
• Arizona Fingerprint Clearance card (program specific).

Responsibilities

• Enhance the likelihood of success, identify and reduce barriers for clients and connect them with needed services and resources.
• Map out the most appropriate next steps for the individual who may need assistance in accessing various services to achieve ongoing success.
• Work with a multi-disciplinary team of internal and external stakeholders.
• Document and track required patient interactions according to AAC, ADHS/DBHS, CARF, Community Bridges Policies and Procedures.
