Job summary

Community Bridges, Inc. in Phoenix is seeking a Navigator II to support clients by connecting them with needed services and resources. The role includes outreach, engagement, case management and ongoing wraparound supports within a multidisciplinary team.

The Navigator II will document and track interactions in compliance with AAC, ADHS/DBHS provider guidelines, CARF, and program policies to ensure quality care and continuous engagement.

Qualifications

  • Highschool diploma or GED is required.
  • Associate degree (or higher) in Behavioral Health is preferred but not required.
  • Minimum of six months of recovery from substance use and/or mental health disorders.
  • 1-3 years of health care related work experience preferred but not required.
  • Minimum of ninety (90) days experience as Navigator or Peer Support Specialist.
  • Must possess a current valid driver's license and be able to provide a 39-month MVR.
  • Arizona Fingerprint Clearance card (program specific).
  • Current AZ Driver's License (Valid and in good standing).
  • Clear 39-month Motor Vehicle Record.

Responsibilities

  • Enhance likelihood of client success by identifying and reducing barriers and connecting clients with needed services.
  • Conduct outreach, engagement, and case management with individuals in need of support.
  • Map out next steps for accessing services to achieve ongoing success.
  • Document and track required patient interactions per AAC, ADHS/DBHS, CARF, and program procedures.
