Job summary

Entyre Care is seeking a dedicated Care Manager to lead in-home assessments and care planning across Ohio. You will visit clients and caregivers, coordinate with physicians, and ensure high-quality, compassionate service in a startup environment.

This high-impact role requires 1+ year of elder care experience, a valid driver’s license, and strong proficiency with MS Office. You’ll embody our mission to support families and empower caregivers while maintaining compliance and community engagement.

Qualifications

• Bachelor's degree required; LSW or LISW preferred.
• Minimum 1 year experience with elders or adults with disabilities.
• Valid driver's license and automobile insurance; travel within Ohio.
• Experience in case management, care planning, and healthcare assessments.
• Proficiency in MS Office (Excel/Word/PowerPoint/Teams).

Responsibilities

• Home visits to assess and create personalized care plans for clients and caregivers.
• Train and support caregivers, ensuring they can meet loved ones' needs.
• Monitor and report on clients’ health, coordinating with primary care physicians and community resources.
• Facilitate smooth transitions and ongoing care for clients.
• Maintain compliance with relevant regulations and standards.
• Participate in community events to raise awareness and enroll new caregivers.
• Embody Entyre Care’s mission, values, and culture as a role model for the team.
