CareStar, Inc. is seeking a Case Manager to support the Ohio Home Care program across multiple counties. You will assess needs, develop personalized care plans, and connect clients with essential services while working within a mission-driven team dedicated to improving lives.

Responsibilities include eligibility evaluations, coordinate services across systems, and advocate for participants with a focus on quality, confidentiality, and regulatory compliance.

Qualificações

• Bachelor’s degree required or equivalent clinical degree.
• Licensed social worker or nursing license as applicable.
• Minimum 1 year of home/community-based services experience.
• Experience with Medicaid, behavioral health systems, and local resources.

Responsabilidades

• Conduct face-to-face eligibility evaluations and assessments (e.g., Adult Needs and Strengths Assessment).
• Work within 8:00 a.m. to 5:00 p.m. duties including phone calls, home visits, and emails.
• Travel to individuals' homes (up to 2 hours each way).
• Develop person-centered care plans and health/safety actions; report to regulators; file incident reports as needed.
• Coordinate services across behavioral health, medical, and community systems.

Conhecimentos

LSW/LISW/LPN/RN license

Formação académica

Bachelor’s degree in Social Work, Counseling, Psychology, Nursing, or a related field
