You will maintain ongoing support for patients, orient them to the center, and collaborate with an interdisciplinary team to navigate care options.

The role emphasizes communication with departments, knowledge of Medicare/Medicaid, and building partnerships with healthcare partners to deliver appropriate care.

Qualifications

• Bachelor’s degree in a related field.
• Associate’s degree with 3 years experience accepted.
• Strong communication and navigation skills in healthcare required.

Responsibilities

• Coordinate pre-admission and admission processes with patients and families.
• Verify patient room readiness prior to admission.
• Maintain contact with families and provide support during transition.
• Provide visits to new admissions to ensure a positive experience.
• Orient patient to center environment.
• Participate in Interdisciplinary Team to assist in navigation needs.
• Coordinate back-up plans for outreach and admissions for nights/weekends.
• Develop partnerships with healthcare partners for appropriate care settings.
