Job summary

Billings Clinic is seeking a Patient Care Navigator for Supportive and Palliative Care to advocate for patients and coordinate care across the continuum, inpatient and outpatient settings. You will work with the multidisciplinary team to ensure timely access, effective communication, and adherence to care plans.

The role requires nursing education (BSN preferred for some applicants) and Montana/Wyoming RN licensure, with 2 years of clinical experience; 5 years of service line experience

Qualifications

• Graduate of an accredited school of nursing.
• Current Montana/Wyoming RN license.
• Two (2) years clinical practice experience, 5 years of service line experience preferred.
• BSN required for candidates with fewer then 5 years of registered nursing experience.

Responsibilities

• Coordinate patient care across inpatient and outpatient settings to ensure seamless flow.
• Schedule coordination and follow-up communication with patients, outside facilities and referring providers.
• Collaborate with the multidisciplinary team to optimize clinical outcomes.
• Maintain accurate documentation and records for accreditation and care continuity.
