Northland Health Centers is seeking a Patient Resource Coordinator to help patients overcome barriers and access care. The role coordinates enabling services and connects families to internal and community supports, with limited CNA scope tasks to support efficient visits.

The position requires a North Dakota CNA certification; CAC certification is preferred or obtainable within 90 days, with occasional travel to multiple locations and some evening/weekend availability.

• Current North Dakota CNA certification required.
• CAC certification preferred or ability to obtain within 90 days.
• Willingness to travel; occasional evening/weekend availability.

Responsabilités

• Link patients to community resources, financial assistance, and insurance coverage options.
• Coordinate with medical, dental, behavioral health, care management, and patient services teams to improve continuity of care.
• Support patient navigation and reducing obstacles to treatment.
• Assist with patient flow and basic clinical tasks within CNA scope.
