Job summary

True Health is seeking a Care Coordinator at its Southside location to support patients and families with educational information related to treatment plans, medications, and continuing care requirements. The role focuses on coordinating care, communicating across the health team, and empowering patients to participate in decision making to improve health outcomes.

Qualifications

• Minimum of a High School Diploma.
• One (1) year minimum experience in care coordination / case management (preferred).
• Experience in Behavioral Health or HIV care coordination is a plus.
• Computer competency is required.
• Bilingual is a plus (English / Spanish).
• Certified or Registered Medical Assistant (preferred) is a plus.

Responsibilities

• Plan with patient, family, primary care provider, support staff, payers, and community resources to maximize healthcare outcomes.
• Facilitate communication and coordination between healthcare team members and patient to minimize fragmentation.
• Educate patients and families about treatment options, resources, and insurance benefits.
• Perform telephonic and face-to-face care coordination tasks and monitor treatment plans and referrals.
• Assist with scheduling, referrals, and documentation to ensure timely care and next steps.
