Prosano Health Solutions, in partnership with Blue Cross Blue Shield of Arizona, is seeking a Clinical Care Guide to join our on-site care team. You will manage end-to-end referral testing navigation and support the care team with patient journeys toward improved health outcomes.

This role requires 100% on-site work at the care center location and may involve movement between clinics. Strong interpersonal skills, BLS certification, EMR experience, and a commitment to high-value care are

Qualifications

High school diploma or GED is required.
BLS certification is preferred or required.
Experience with Electronic Medical Records (EMR) systems.
Customer service orientation and effective communication.

Responsabilités

Deliver end-to-end referral testing navigation and support the care team.
Lead and partner with the care team to support patients on their journey toward better health.
Educate members in real time and schedule specialist and diagnostic referrals.
Follow up on appointments to ensure continuity of care and outcomes.
Contribute to quality programs and patient outreach for gaps in care.
Support members with chronic conditions through education and reminders.
Manage service and schedule requests on behalf of members.
Provide referral information and requirements to the Care Team.
Assist in specialist relationship management and related duties.
