Prosano Health Solutions is hiring a Clinical Care Guide to join our first-class care team. You will be responsible for end-to-end referral navigation, coordination of specialist and diagnostic testing, and partnering with the care team to support patients in their journey toward better health.

You will educate and assist members in scheduling, follow-up, and care coordination while upholding privacy and compliance in a dynamic health setting.

Qualifications

• - BLS certification is required.
• - AZ fingerprint clearance card is required.
• - High school diploma or GED is required.
• - EMR experience and familiarity with healthcare software are preferred.
• - Demonstrated empathy, confidentiality, and exceptional communication.

Responsibilities

• The Clinical Care Guide will support members in-person and virtually with education and scheduling of referrals.
• Coordinate and follow up on specialist and diagnostic appointments.
• Help improve health outcomes and cost savings through patient care.
• Explain referrals and navigate end-to-end referral processes.
• Support quality programs and patient outreach to close care gaps.
• Assist in managing chronic conditions with education and reminders.
• Lead service requests and provide referral information to the Care Team.
• Maintain patient privacy and ensure compliance during interactions.
• Handle on-site duties; some clinics movement may be required.
