Job summary

Prosano Health Solutions, in partnership with Blue Cross Blue Shield of Arizona, seeks a Clinical Care Guide to support patients with referrals and care navigation. You will work on-site to ensure accurate scheduling, follow-through, and close coordination with care teams.

You will play a key role in improving health outcomes while maintaining confidentiality and delivering exceptional patient-centered service in a dynamic health setting.

• Manage end-to-end referral processes as needed.
• Support quality programs and patient outreach to close care gaps.
