Job summary

Thresholds is seeking a Community Support Specialist in Chicago to provide mental health and substance use support within clients’ communities. You will coordinate care, connect clients to resources, and help remove barriers to access while adapting to each client’s schedule.

Role involves driving clients to appointments and assisting with daily living skills, with opportunities for clinical supervision and professional development within our collaborative team.

Qualifications

• Must be able to communicate effectively and build relationships with clients.
• Strong time management and organizational skills are essential.
• Applicants must be at least 21 years old with a valid driver's license and daily access to a car.
• Willingness to transport clients as needed.
• Multiple education paths qualify for this role.
