Mountain Area Health Education Center - MAHEC seeks a Care Navigator to work within MAHEC's Population Health Administration embedded in primary care teams, coordinating resources to support patients toward optimal health.

The role partners with clinical teams, behavioral health, and community providers to meet performance goals and deliver patient-centered care and self-management support.

Qualifikationen

• Experience documenting histories and care plans in EHR and care management platforms.
• Ability to engage patients and families to support self-management.
• Strong communication with care teams and community partners.
• Team-based care approach to screening for depression and social determinants of health.

Aufgaben

• Engages patients and families to document histories and implement person-centered care plans.
• Develops relationships to facilitate engagement during in-person visits, group visits, and virtual encounters.
• Uses motivational interviewing to gather clinical and psychosocial information and coach patients toward self-management.
• Identify barriers to care and coordinate with Providers, Nursing care managers, social workers to meet goals.
• Communicates effectively with caregivers to achieve targeted outcomes.
• Documents activities, service plans, and results in EHR and care management platform.
• Refers patients to educational resources regarding health care, programs, and services.
