Job summary

Mass General Brigham Community Physicians, Inc. in Massachusetts is seeking a Community Health Worker to provide health education, outreach, and care coordination to individuals and families in the community.

The role partners with healthcare providers to ensure patients receive needed services, conducts needs assessments, and supports navigation of the healthcare system with flexible, hybrid in-community and remote work.

Qualifications

• Bachelor's degree in Public Health, Social Work or related field required.
• 2+ years of experience in community health outreach, health education, or related field.
• Community Health Worker Massachusetts License preferred.
• Experience with Social Determinants of Health (SDOH), housing, food insecurity, transportation, utilities, employment, childcare assistance programs preferred.
• Experience helping patients apply for state/federal programs/benefits (i.e. SSDI, SSI, SNAP) and advocating for the patient with appeals preferred.
• Knowledge of transportation resources (PT1) preferred.
• Bilingual skills preferred.

Responsibilities

• Provides advanced health education, outreach, and care coordination services to individuals and families in the community.
• Works closely with healthcare providers to ensure that patients receive appropriate care and support services to improve health outcomes.
• Conducts in-depth assessments of patient needs, goals, and barriers to achieving good health outcomes.
• Provides advanced health education and coaching to individuals and families to promote healthy behaviors and self-management of chronic conditions.
• Collaborates with healthcare providers to develop and implement care plans for patients with complex health needs.
• Assists patients in navigating the healthcare system and accessing appropriate services.
