Join to apply for the Care Navigator role at Child & Family Services, Inc.

Child & Family Services is looking for a full-time Care Navigator to join our Community Behavioral Health Center (CBHC) team in New Bedford, MA. The Care Navigator provides behavioral health services, including access to same-day or next-day services. The role acts as a liaison to connect clients and families with appropriate services.

Schedule and Compensation:

• Monday - Friday, with availability until 8PM on Tuesdays (later start), $1/hour language differential (eligible languages include Cape Verdean Creole, Haitian Creole, Portuguese, Spanish, or ASL).
• Saturdays & Sundays shift differentials: $2.50/hour for 1st shift, $4.00/hour for 2nd shift.

Qualifications:

• Bachelor’s Degree in Social Work, Sociology, Psychology, or a related human services field.
• At least one year of experience providing mental health or substance use services to youth, adults, or families.
• Demonstrated cultural humility and understanding of ethnic, cultural, and linguistic considerations.
• Bilingual individuals preferred to support community needs.

Responsibilities:

• Facilitate walk-in process with clients.
• Complete pre-screening assessments, identify client needs, and determine service options.
• Use collaborative documentation, schedule appointments, and make referrals.
• Support transitions of care, engage in care coordination, and facilitate wellness and recovery.
• Employ evidence-based interventions, develop service plans, and provide advocacy and psychoeducation.
• Support crisis intervention, facilitate warm handoffs, and build community resource connections.
• Participate in supervision and training, maintain records, and ensure high-quality, culturally competent care.
