Position Overview

Centene is transforming healthcare for our 28 million members by providing quality, cost-effective care. This remote position requires residency in Mississippi and offers competitive benefits, including flexible work arrangements.

Responsibilities

1. Assess member needs, barriers to care, and resources, then develop and facilitate personalized care plans.
2. Coordinate with providers, community resources, and members to ensure timely access to services.
3. Support members and caregivers with psychosocial and resource guidance.
4. Monitor progress and collaborate with healthcare providers to revise care plans as needed.
5. Maintain accurate documentation to ensure compliance with regulations.
6. Conduct on-site assessments and provide educational support as appropriate.
7. Perform additional duties as assigned, adhering to all policies and standards.

Qualifications

• Bachelor’s degree with 2-4 years of related experience, or clinical licensure (e.g., Nursing with license).
• Preferred: Degree in social work, sociology, psychology, or public health; LSW or LMSW certification.

Additional Information

Pay range: $22.50 - $38.02/hour. Benefits include health insurance, 401K, tuition reimbursement, paid time off, and flexible schedules.
