The Senior Living Care Navigator is a non-clinical, non-licensed healthcare resource conducting telephonic outreach to members, addressing requests for assistance, and partnering with advanced practice clinicians (APCs) and nurses (RNs) to support member care coordination.

This full-time position (40 hours/week) operates Monday to Friday. Flexibility to work any of our 8-hour shifts during 8:30am - 5:00pm PST is required. Occasional overtime may be necessary.

Training consists of 3 to 4 weeks, scheduled based on your availability or discussed on your first day. The initial week may have different hours.

You will have the flexibility to telecommute from anywhere within the U.S.

Primary Responsibilities
  • Telephonic outreach to new and existing members to introduce or review the care team and plan
  • Support care coordination activities and administrative tasks alongside APCs
Job Duties
  • Guide members on utilization of plan benefits such as transportation and OTC products
  • Complete telephonic assessments
  • Coordinate medical appointments and health services
  • Facilitate communication between members and the care team
  • Provide community resource information to address social needs
  • Perform other duties as assigned
Care Coordination Tasks
  • Scheduling PCP and specialist appointments
  • Arranging diagnostics, dental, vision, hearing, DME, home health, and transportation services

  • Maintain appropriate tone and conduct in challenging situations

Required Qualifications
  • High School Diploma / GED or equivalent experience
  • Must be 18 or older
  • At least 2 years experience in a clinical setting involving patient and team communication
  • Experience working with cross-functional teams
  • Strong telephonic and written communication skills
  • Proficiency with EMRs
  • Understanding of medical terminology and team collaboration
Preferred Qualifications
  • Over 3 years of clinical or medical record experience
  • Experience with Microsoft Office applications
  • Background as CNA, Medical Assistant, or LPN
  • Strong organizational skills and ability to multitask
  • Reside in Washington or Oregon
Telecommuting Requirements
  • Secure handling of sensitive documents

  • Reliable high-speed internet connection
  • Adherence to company telecommuting policies

The hourly pay ranges from $19.86 to $38.85, depending on experience and other factors. Benefits include comprehensive packages, incentives, stock options, and 401k. The application deadline is at least 2 business days from posting or until enough candidates are received.
