Job summary

Healthfirst seeks a Care Coordinator to support members requiring assistance navigating the health care system. The role may be aligned to a Care Management team or a call center, performing care management services accordingly.

The position requires CMA/CNA/RMA or equivalent medical training, HS diploma, and strong communication skills. Prefer Associate’s Degree and prior call center or healthcare experience. Salary ranges vary by location, with a nationwide health benefits package offered.

Qualifications

• Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), or Registered Medical Assistant (RMA) or equivalent medical training/work experience.
• HS diploma or GED required.
• Strong verbal and written communication with good grammar, spelling and punctuation.

Responsibilities

• Handle incoming calls from members regarding benefits, care coordination, program inquiries, and connect to care manager.
• Manage assigned member caseload in care management, coordinate with interdisciplinary team, and monitor member needs.
• Perform outreach calls to confirm services are in place.
• Other duties as assigned.
