Job summary

Woodlands Senior Living in Maine is seeking a Care Navigation Specialist to engage with families by phone and email, explain care options, schedule tours, assist with applications and gather required documentation. You will build trusted relationships as you guide families through the pre-admission process and coordinate with the care team.

Qualifications

Responsibilities

• Manage leads using leads management software.
• Represent the company at trade shows and expos.
• Schedule community tours and coordinate with Administrators/Executive Directors.
• Receive referrals from external sources and maintain relationships with referral sources.
• Respond to inquiries from families and prospective residents.
• Conduct community outreach and attend senior-focused events.
• Assist prospects through the pre-admission process with empathy.
• Maintain expertise in MaineCare program and application process.

What you will do as a Care Navigation Specialist:

• Be responsible for leads management program and utilization of leads management software
• Serve as company representative for trade shows and expos
• Schedule community tours to Administrator/Executive Director’s calendar
• Receive incoming referrals from external referral sources, maintaining excellent relationships with those referral sources
• Receive incoming inquiries from family members, prospective residents, and those acting on behalf of prospective residents
• Conduct community outreach to referral sources and attend senior-focused community events
• Operate with empathy and helpfulness when assisting prospects through the pre-admission process
• Maintain expertise in Long Term Care MaineCare program and application process

Requirements

What you’ll bring to the role:

• Previous experience in a senior living community or nursing home devoted to the care of aging community members preferred.
• Ability to provide high-level customer service that is both empathetic and thorough.
• Ability to comfortably manage competing priorities and move between different tasks efficiently.
• Understanding of the aging process and/or medical terminology.
• Ability to multitask, particularly by performing data entry while on the phone with a customer.
• A current valid license to operate a vehicle in the state of Maine and a driving record that is satisfactory to the company is required.
• Must complete a criminal history record check to the satisfaction of the company.
