What You'll Do

As a Care Navigation Specialist, you will:

  • Serve as a compassionate resource for prospective residents and family members seeking senior living and healthcare guidance.
  • Respond to inquiries via phone, email, and other communication channels with professionalism, empathy, and urgency.
  • Guide families through the pre-admission process by explaining available care options and next steps.
  • Coordinate and schedule community tours with Administrators and Executive Directors.
  • Assist families with applications, required documentation, and MaineCare-related processes.
  • Manage and track leads through our customer relationship management and lead management systems.
  • Build and maintain strong relationships with referral partners, healthcare providers, and community organizations.
  • Receive and coordinate incoming referrals from external healthcare and community sources.
  • Represent Woodlands Senior Living at healthcare events, trade shows, community outreach activities, and senior-focused programs.
  • Develop expertise in long-term care services, resources, and support programs available throughout Maine.
  • Deliver an outstanding customer experience while helping families make informed care decisions.

Requirements
We're seeking someone who:
  • Has a genuine passion for helping seniors, families, and individuals navigate healthcare and support services.
  • Is interested in building a long-term career in healthcare, community health, social services, senior living, or care coordination.
  • Demonstrates strong communication and relationship-building skills.
  • Provides exceptional customer service with empathy, professionalism, and attention to detail.
  • Can effectively manage multiple priorities in a fast-paced environment.
  • Is comfortable using technology and performing accurate data entry while speaking with customers.
  • Possesses an understanding of healthcare terminology, aging services, senior care, or human services concepts, preferred.
  • Has previous experience in senior living, healthcare, customer service, community outreach, human services, or related fields, preferred.
  • Holds a valid Maine driver's license with a satisfactory driving record.
  • Successfully completes a criminal background check.
