• Independently collects, compiles, and analyzes Community Care program performance data.
• Monitor weekly and monthly reports and ensure timely, accurate deliverables.
• Provide administrative and analytical support for projects, tracking progress and maintaining docs.
• Coordinate routine aspects of special projects and ensure deadlines are met.
• Act as information resource on procedures, guidelines, and reporting; update training materials.
• Support HR, budget, property, and equipment processes for Community Care and Referral Coordination.
