Sacramento Native American Health Center, Inc. is seeking an ECM Care Manager to deliver direct services and care coordination as part of a multidisciplinary team in Hanford, CA.

You will assess needs, develop ICP/HAP, advocate for clients, and ensure access to health and social services that support wellness and independence. The role involves in-person work, delivering services in-office, via telehealth, and in the field, while coordinating with providers and community resources.

Qualifications

• Must have prior experience in care coordination or case management.
• High school diploma or equivalent is required.
• Familiarity with local community resources and supports is essential.
• Strong interpersonal skills, empathy, active listening, good judgment, and teamwork are required.

Responsibilities

• Assess physical health, mental health, SUD, oral health, trauma history, social supports, housing, employment, and community services.
• Develop and oversee Individual Care Plans (ICP) and Health Action Plans (HAP).
• Provide in-office, telehealth, and field-based services to meet clients where they are.
• Coordinate care with providers (with consent) and ensure continuity from assessment through discharge.
• Apply evidence-based approaches (Motivational Interviewing, Trauma-Informed Care, Harm Reduction).
• Document services clearly and compliantly in the EHR.
