Job summary

Telligen is seeking a Care Manager Supervisor to lead program staff in Illinois, ensuring compliance with URAC accreditation and state requirements. The role involves supervising day-to-day operations, coaching staff, and developing training protocols for ongoing staff education.

The position focuses on person-centered care planning, transitions of care, and quality improvement activities within the Tailored Care Management program, with emphasis on leadership and collaboration across teams.

Qualifications

• Master's degree with 2+ years case management experience, including elderly/disabled populations.
• Illinois RN/OT/LCSW/LCPC/LMFT license is required.
• 3-5 years clinical experience.
• Minimum 5 years leadership experience.
• Experience with developmental disabilities, serious mental illness, and complex medical needs.
• Valid driver's license.

Responsibilities

• Supervises day-to-day operation of staff to ensure compliance with program protocols, state requirements and URAC accreditation standards as applicable.
• Ensures timely and satisfactory completion of contract/program operations.
• Selects, develops, and manages employees to achieve individual, team, and organizational success.
• Ensures employee accountability through routine observation, performance measurement, and feedback.
• Performs audits or quality monitoring of staff to provide feedback on strengths and opportunities for improvement.
• Develops training protocols for staff and patient education tools for orientation and ongoing education.
• Stays abreast of health care trends and changes.
