Job summary

Bon Secours Mercy Health is seeking a Social Worker Care Manager at St. Mary’s Hospital Richmond. The role focuses on discharge planning, patient advocacy, and coordinating resources across hospital and community settings.

Responsibilities include identifying patients in need, documenting plans, and guiding transitions of care while ensuring ethical, patient-centered care in a team-based environment. Requires BSW, LICSW preferred in VA, and relevant clinical experience.

Qualifications

• Bachelor of Social Work is required.
• Master’s degree in social work or healthcare related field is preferred.
• Licensed as a Social Worker in state of practice is required, VA preferred.
• 1 year of experience in clinical setting is required.
• 3 years of experience in acute care clinical setting is preferred.

Responsibilities

• Identify and prioritize patients in need of social services, planning with patient/caregivers and healthcare team.
• Complete and maintain clear, timely documentation in the patient record.
• Ensure transition of care planning reflects patient/family goals to the care team.
