Job summary

Your Health in Walterboro, SC is seeking a Clinical Scheduler / Care Manager to serve as the central scheduling and operational coordinator for a single interdisciplinary care team. This role ensures patients receive the appropriate number and type of in-person encounters under our proactive, value-based care model, with daily in-office work.

The position is full-time, 8-hour shifts (8:00AM-5:00PM), Monday-Friday, located in the Walterboro office, supporting ongoing patient care and coordination

Qualifications

• Experience in medical/healthcare scheduling or administrative role.
• Familiar with medical terminology and insurance verification.
• Strong organizational and time management abilities.
• Excellent communication and relationship-building skills.

Responsibilities

• Register, verify, and timely schedule patients according to the Company’s care model.
• Ensure new patients are tracked and followed until scheduled.
• Schedule appointments based on care model, including follow-ups with providers.
• Optimize schedules across all disciplines within the care team.
• Schedule patients after discharge from in-patient facilities.
• Ensure timely TCM appointments within required windows.
• Maintain thorough documentation of scheduling activities.
• Utilize multiple software systems to support patient follow-up and outcomes.
• Interact with patients via email, portal, phone, AthenaText, and in-person to gather info, confirm appointments, and provide pre-appointment instructions.
