Your Health in Rock Hill, SC is seeking a Clinical Scheduler to support our Care Manager, coordinating all in‑person visits for a single interdisciplinary care team. The role serves as the central scheduling and operational liaison to ensure appropriate encounters under our value‑based care model, with daily work in the Rock Hill office from 8:00 AM to 5:00 PM, Monday through Friday.

• High school diploma or equivalent required; an Associate's or Bachelor's in Healthcare preferred.
• Experience in medical scheduling or administrative role is desired.
• Familiarity with medical terminology and insurance verification is preferred.

Responsibilities

• Register, verify and timely schedule patients according to the care model.
• Track new patients and ensure follow-up scheduling.
• Schedule appointments based on availability, urgency, and patient preferences.
• Optimize all disciplines' schedules and coordinate discharges from in-patient facilities.
• Ensure prompt scheduling of TCM appointments within required windows.
• Maintain strong documentation and utilize multiple software systems to support scheduling.
• Communicate with patients via email, portal, phone, or in person to gather information and confirm appointments.
