Volunteers of America Delaware Valley is seeking a dependable, client-focused Care Manager to support daily facility operations and client needs, reporting to the Program Manager. The role is part of the treatment team and helps ensure a safe, structured environment through client accountability, documentation, medication monitoring, and day-to-day support.

Qualifications

• High school diploma or equivalent is required.
• 2+ years of related work experience in a relevant field.
• Direct experience working with homeless populations is preferred.
• Experience with individuals with substance use and mental health challenges is desired.
• Strong written and verbal communication skills are essential.
• Ability to interact professionally with clients.
• Valid driver’s license and ability to meet agency driving standards.
• Knowledge of HIPAA Privacy and Security Rules is important.
• Bilingual abilities are a plus.

Responsibilities

• Support ongoing facility safety and security.
• Complete rounds, document conditions, and conduct head counts.
• Conduct shift-change communication to incoming staff.
• Review census and relay information.
• Monitor sign-in/sign-out and address late/absent clients.
• Orient new clients to rules and procedures.
• Coordinate meal distribution and related concerns.
• Inventory client personal property and documentation.
• Monitor client medications and logs in the Medication Log.
• Schedule and document medical appointments.
• Ensure chores are completed and cleaning supplies are stocked.
• Conduct facility inspections and searches per policy.
• Ensure visitors sign in and out, log visits.
• Log phone calls and respond to inquiries appropriately.
• Conduct monthly fire drills and safety checks.
• Maintain daily logs of clients and events.
• Transport/escort clients as needed.
• Document disciplinary incidents and complete paperwork timely.
• Maintain files and complete core curriculum training obligations.
• Maintain confidentiality and comply with policies.
