Job summary

Iberia Medical Center in New Iberia, LA is hiring a Registered Nurse - Care Manager who, under the supervision of the Unit Director and with the authority of the Chief Nursing Officer, will apply the nursing process to care for patients across age groups. The role emphasizes care coordination with patients, physicians, and referral sources.

Responsibilities include ongoing clinical reviews to ensure appropriate level of care, documentation in the patient system, and ensuring effective discharge

Qualifications

• Graduate of state approved School of Nursing.
• Current RN licensure in Louisiana.

Responsibilities

• Be available by telephone during working hours.
• Act as liaison between patients, physicians, and referral sources.
• Be knowledgeable of all assigned patients.
• Conduct ongoing clinical reviews to ensure appropriate level of care and necessity of continued stay.
• Report clinical reviews to third party payer sources as required/requested.
• Post authorizations into computer as needed.
• Make appropriate referrals as ordered per MD.
• Ensure discharge plans are in place for patient safety in the post-acute setting.
• Actively participate in interdisciplinary rounding meetings.
• Comply with all compliance expectations.
• Knowledgeable of CMS Mandated Forms.

EDUCATION

LICENSURE/CERTIFICATIONS
