Pardee UNC Health Care is seeking a dedicated Care Manager - RN to provide comprehensive assessment, planning, and evaluation to support quality patient management and satisfaction. The role emphasizes continuity of care, cost effectiveness, and discharge planning within a multidisciplinary environment.

The position requires NC or compact-state RN licensure, 3 years of acute care experience, and BLS certification; BSN preferred.

Qualifications

• Must be licensed to practice as a Registered Nurse in North Carolina or a compact state.
• Three years of recent clinical experience in an acute care setting/home health/long-term care.
• Basic Life Support (BLS) certification.

Responsabilités

• Reviews admissions and prioritizes patients; documents assessment and plan of care in the EMR.
• Collaborates with multidisciplinary team to address discharge barriers and psychosocial concerns; coordinates family meetings as needed.
• Educates patients and families on community resources and coordinates referrals to post-acute providers.
• Coordinates with units to monitor patient progression; alerts care team to discharge readiness and adjusts plans as needed.
• Works with Utilization Manager and Social Worker to problem-solve complex cases and escalate as needed.
• Maintains documentation for third-party payers and other agency requirements.
• Performs admission and continued stay reviews to determine hospital admission appropriateness.
