Job summary

Providence Medical Foundation is seeking a Team Based Care (TBC) Manager RN to independently care for a group of patients using the nursing process in a safe, therapeutic, and efficient manner. You will perform shared visits with providers and independent RN visits following standardized procedures and documented workflows within a Team Based Care Model.

Qualifications

• Graduate from an accredited school of Nursing.
• California Registered Nurse License upon hire.
• Two (2) years Professional Nursing Experience.
• Bachelor's Degree in Nursing (preferred).
• One (1) year experience in ambulatory care nursing (preferred).

Responsibilities

• Provide Team Based Care management for a patient group and coordinate with providers.
• Conduct shared patient visits with providers and independent RN visits using standardized procedures.
• Collaborate with the care team to coordinate and execute the plan of care and document workflows.

Skills

Two years nursing experience

Ambulatory care experience (preferred)

Team-based care collaboration

Education

Graduate from an accredited school of Nursing

Bachelor's Degree in Nursing (preferred)

Job description
