Providence Jobs in Mission Viejo, CA is seeking a Team Based Care (TBC) Manager RN who uses the nursing process to care for a group of patients safely and efficiently. This role conducts shared patient visits with providers as well as independent RN visits using standardized procedures and documented workflows.

The TBC RN collaborates with other team members to coordinate and execute the plan of care, and demonstrates initiative in the delivery of patient care in a Team Based Care Model,

Qualifications

• Graduate from an accredited school of Nursing.
• California RN license required upon hire.
• Two years of professional nursing experience.
• Bachelor's degree in nursing preferred.
• One year of ambulatory care nursing experience preferred.

Responsibilities

• Independently uses nursing process to care for patients safely and efficiently.
• Collaborates with providers for shared patient visits and conducts independent RN visits.
• Coordinates and executes the plan of care with the care team.
• Demonstrates initiative in delivering care in a Team Based Care Model.
