Providence Medical Foundation seeks a Team Based Care (TBC) Manager RN to lead patient care using the nursing process, delivering both shared visits with providers and independent RN visits within standardized protocols.

You will coordinate the plan of care with the care team, demonstrating initiative in a Team Based Care Model and embodying Providence values in every interaction.

Formación

• Graduate from an accredited school of Nursing.
• California Registered Nurse License upon hire.
• Two (2) years professional nursing experience.

The Team Based Care (TBC) Manager RN independently uses the nursing process to care for a group of patients safely, therapeutically, and efficiently. This role conducts shared patient visits with providers as well as independent RN visits using standardized procedures/protocols and documented workflows. The TBC RN collaborates with other team members to coordinate and execute the plan of care. Demonstrates initiative in the delivery of patient care in a Team Based Care Model. Actions will reflect the Providence core values.

Required Qualifications:

• Coursework/Training: Graduate from an accredited school of Nursing.
• California Registered Nurse License upon hire
• Two (2) years Professional Nursing Experience.

Preferred Qualifications:

• Bachelor's Degree in Nursing.
• One (1) year experience in ambulatory care nursing.
