Job summary

The Inpatient Registered Nurse (RN) Care Manager at Providence Holy Cross Medical Center in Mission Hills, CA, is a full-time role with day shift hours. You will assess and plan, coordinate care, manage resources, and oversee discharge planning to ensure appropriate care across the continuum.

The ideal candidate has an active California RN license, an ADN, 2 years acute care experience, and at least 1 year in care management or utilization review.

• California Fire and Life Safety Card within 30 days of hire.
• 2 years acute care experience in a Medical-Surgical or Inpatient setting.

• Completion of TIPS program or Case Management Orientation Program.

Care Manager RN at Providence Holy Cross Medical Center in Mission Hills, CA. This position is Full-time and will work 8-hour, Day shifts.

The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient centric care management services for at risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, resource utilization management and/ or review, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management RN is to ensure the use of appropriate healthcare resources throughout the continuum, so that the care provided is the right care, at the right time, in the right setting.

Required qualifications:

• Associate's Degree in Nursing degree/diploma upon hire
• Upon hire: California Registered Nurse License.
• within 30 days of hire: California Fire and Life Safety Card - National Organization
• 2 years Acute care experience in a Medical Surgical or Inpatient setting.
• 1 year
• Experience in care management or utilization review in any setting, OR
• Upon hire successful completion of TIPS program or Case Management Orientation Program

Preferred qualifications:

• Bachelor's Degree in Nursing or higher within 3 years of hire
