Job summary

Advance Care Alliance New York is seeking a Care Manager to oversee and coordinate services for people with intellectual and developmental disabilities. The role focuses on developing Life Plans with an interdisciplinary team to meet medical, behavioral health, and community needs, while advocating for members’ goals.

The position emphasizes comprehensive assessments, care coordination, and regional travel up to 75%.

Qualifications

• Bachelor's degree or RN license with relevant experience, or master's with relevant experience.
• Fluent understanding of community cultural traditions, norms, and practices of Members and families.
• Integrity and commitment to serving people with I/DD and families.
• Excellent interpersonal, public speaking, and written communication skills.
• Ability to work autonomously.
• Professionalism, respect, and teamwork ability.

Responsibilities

• Deliver person-centered care management services in compliance with standards and SOPs.
• Complete comprehensive assessment/reassessment processes.
• Identify gaps, refer when appropriate, and advocate for goals.
• Develop, implement, and monitor Life Plans with a interdisciplinary team.
• Address conflicts in planning and resolve them timely.
• Complete all required service documentation and billing-related records.
• Maintain eligibility through annual determinations and HCBS waiver enrollment.
• Identify benefits access (Medicaid, Social Security, SNAP) when eligible.
• Maintain HIPAA-compliant information sharing in the EHR.
• Coordinate access to medical, behavioral health, and community services.
• Coordinate preventive and health promotion services as needed.
• Coordinate transitions from inpatient to other settings and Adult transition.
• Use EHRs and telehealth to deliver care management.
• Attend meetings, trainings, and supervision; complete trainings.
