Job summary

Advance Care Alliance New York is seeking a Care Manager in Manhattan to oversee access to services for members with IDD, coordinating Life Plans and working with families, providers, and the interdisciplinary team.

You will advocate for members, complete assessments, and monitor care with a focus on quality, person-centered outcomes. Travel within the service area is required; remote work is not available.

• Ensure timely, accurate documentation and billing-related records.
