Qualifications

• RN license to practice in Florida (or LPN as alternative).
• Proficiency with EHR systems and basic computer applications.

Responsibilities

• Assess member needs and develop person-centered care plans.
• Coordinate services across care settings and with providers.
• Monitor progress and adjust plans to ensure continuity of care.
• Serve as primary contact for members and authorized representatives.
• Educate on available resources and support self-advocacy.
• Maintain accurate documentation in compliance with policies.
