Qualifications

• High school diploma or equivalent; 2-4 years in healthcare navigation or coordination.
• Experience with appointment coordination, referrals, and patient communication.
• Experience with electronic health records and scheduling/case management systems.
• Strong problem solving, documentation, and follow through.
• Bilingual abilities or CHW training preferred based on patient needs.

Responsibilities

• Coordinate patient access activities including appointment scheduling, referrals, and care team communication.
• Identify barriers to care access and assist with logistics, finances, or operational issues.
• Resolve scheduling or service coordination issues within procedures and escalate when needed.
• Communicate next steps clearly to patients, families, and care teams.
• Track outreach, pending actions, and completed tasks in designated systems.
• Partner with clinical, operational, payer, and community teams for timely follow-up.
• Escalate clinical concerns to licensed staff or leadership as required.

Job Description Summary

We are excited to announce an opening for a Care Manager. A Care Manager supports patients and families through routine and moderately complex navigation needs by coordinating appointments, care team communication, community resources, and follow-up activities. Resolves common access barriers, interprets standard workflows, and helps patients understand next steps across care settings, payer requirements, and internal service channels. The Care Manager carries out defined clinical support processes under moderate supervision, applying working knowledge to tasks that occasionally require interpretation or deviation from standard procedures. Decisions are procedure-driven and impact day-to-day patient interactions and team workflow. This job handles routine and moderately non-routine situations, communicating clearly with patients, members, and clinical staff.

How will you make an impact & Requirements

Key Responsibilities

• Coordinate patient access activities including appointment scheduling, referral follow-up, resource connection, and care team communication.
• Review patient information to identify common logistical, financial, social, or operational barriers to care access.
• Resolve moderately non-routine scheduling, documentation, or service coordination issues within established procedures.
• Communicate next steps to patients and families in a clear, empathetic, and culturally responsive manner.
• Track outreach, pending actions, completed tasks, and unresolved barriers in designated systems.
• Partner with clinical, operational, payer, and community resource teams to support timely patient follow‑up.
• Escalate clinical concerns, complex access barriers, or situations outside established procedures to licensed clinical staff or leadership.

Qualifications

• High school diploma or equivalent required; 2-4 years of healthcare, patient navigation, care coordination, medical office, call center, or community resource experience.
• Working knowledge of appointment coordination, referral workflows, insurance basics, and patient communication standards.
• Experience using electronic health record, scheduling, care management, or customer relationship management systems.
• Strong problem‑solving, documentation, and follow‑through skills.
• Ability to interpret established procedures and communicate clearly with patients, families, and care teams.
• Bilingual skills or community health worker training preferred based on patient population needs.

Compensation Range

$20.90 to $31.35 The anticipated base salary range represents the Company's good‑faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
